Online Submission Forms

Payment is required for your submission to be reviewed. After completing your submission and if payment is required, please send a check payable to NOHOA to 100 Village Center Drive, Suite 240, North Oaks MN 55127. 
 
NOHOA may require additional documentation as part of the application process depending on the project type. 
  • Lot Survey
  • Site Plan
  • Staking; outline of proposed structure marked on lot (if needed)
  • Trees; mark trees to be removed with orange tape (if needed)
  • Sewer & Well; location marked (if relevant)
 
Important Notes:
  1. When filling out the submission form we will be contacting you using the email you have provided in the submission form. 
  2. Verify that your attachments are connected to your submission. If they are too large the submission form will not accept them and you will need to email them to nohoa@nohoa.org. 
  3. You will be contacted regarding your submission once it has been reviewed. They might ask for more information or send a Zoom Link for the virtual meeting.
